This form should be executed by a prosecuting United States Attorney's Office or federal law enforcement

agency other than the Bureau of Prisons (BOP) when the agency or office perceives a potential threat of

continuing criminal activity by a person who is or will be imprisoned by the BOP. This form has the limited effect

of providing law enforcement information to the BOP for purpose of improving the BOP's ability to ensure

institution security and protect the public. This should be mailed to the BOP's intelligence Section, Room 541,
