Having considered the Petition for Appointment of Guardian and Conservator for Minor and the evidence presented, the Court finds:

1. Petitioner is entitled to file pursuant to A.R.S. §§ 14-5207(A) and -5404.

2. The Court has jurisdiction and venue in this county is proper.

3. Notice was given as required by law.

4. (Minor's name) is __________ years old and unmarried.

5. All parental rights of custody have been terminated or suspended by circumstances or prior court order.

6. No guardian has been appointed for (minor's name) by a Will or any court order.

7. No other proceeding for the appointment of a guardian for (minor's name) is pending in any other court.

9. Pursuant to A.R.S. §§ 14-5206, -5207, and -5410(A), (appointee's name) is qualified to serve as (minor's name)'s guardian and conservator and has priority for appointment.
